How do I get a refill on my medicine?

The best way to get refills on most medications is by calling your pharmacy who can send an electronic request to her office.  If it has been a while since your last visit, she may request you get an appointment as some medications need monitoring.  Best practices mean that you schedule your next visit on your way out of the office if you are on long term medications.  

Where is the office located?

The address of our location is 8241 Rochester Avenue,  Suite 110 in Rancho Cucamonga.   Our office is south of Foothill and in Easternmost part of The Courts which is just north of the Rancho Cucamonga Sports Arena.  We are very close to Quakes' Stadium and about a mile from Victoria Gardens.  We are freeway close to the 15, 210 and 10 freeways.  We also offer telemedicine synchronized audio and video appointments for some visits.

What are the office/phone hours?

We see patients primarily on Tuesdays, Wednesdays, and Thursdays.  The office is open those days from 8:30 am to 5:30 pm but closed for lunch from 1-2pm.  The phones are open 8:30- noon and 2-4:30 pm. We are only open on Fridays from 8:30-1 pm.  We are closed other days.

What care is offered here?

We treat patients from ages 6-106.  We perform Physical exams, Well Woman exams, Childhood exams, and Department of Transportations exams.  We also see patients for Sick visits and treat Chronic Conditions like Diabetes, Hypertension,  High Cholesterol,  Headaches, Infections, Skin lesions, Female Issues and Pains/Strains/Sprains of many types.  

How can I become a patient?

The first step is to call our office to see if your insurance covers care at our office.  If you do not have insurance, we do treat patients on a cash basis.  Our availability to see new patients varies, as well as the cost of care due to inflation costs.

How can I get a Same Day appointment?

We offer sick patients whom are unable to work to call us between 8:30-9:00 am Tuesday through Friday to get on our daily list.  Dr. Leoni will read through the list to determine which patients are most in need of being seen.  If offered a Same Day appointment, we anticipate the patient whom called in will be able to come in or do a telemedicine visit for an appointment offered as they should not be working.   

What if I need to be seen when the office is closed?

There are several Urgent Care centers in our vicinity.   Some Insurance types require the use of a particular place.  Please check with your insurance company. Please call 911 or go to the Emergency Room if you are having: 

Chest pain

Severe Abdominal pain

Bone fracture

Shortness of breath

Severe Headache

Severe Back pain

Heavy bleeding

Allergic Reaction which causes Breathlessness, or Swollen tongue

Fainting

Seizure

Severe Burns

Diarrhea

Head injury
